Summary

The facility failed to maintain accurate and complete medical record documentation for two residents. For one resident, the admission record did not include a diagnosis of diabetes even though the history and physical documented diabetes mellitus, and staff confirmed the resident was receiving Humalog Lispro on a sliding scale for diabetes. During interviews, the LVN, DON, and MDS coordinator all reviewed the resident’s facesheet and history and physical and acknowledged that the diabetes diagnosis was missing from the facesheet and that the diagnosis should have been reflected in the resident’s record and plan of care. For another resident, the admission record and MDS listed bipolar disorder as a diagnosis, but the resident’s history and physical signed by the MD did not include bipolar disorder. The resident’s order summary showed an order for olanzapine with bipolar disorder documented as the indication, but the MD stated the resident did not have a diagnosis of bipolar disorder and that the diagnosis was likely entered by the admitting nurse in error. The MDS coordinator stated the bipolar disorder diagnosis had been included in the MDS and admission record based on the resident’s active olanzapine medication and that the coordinator was acting in good faith that the MD had reviewed the medications and agreed with the diagnosis. The facility policy titled Charting and Documentation required documentation in the medical record to be objective, complete, and accurate. The record review and interviews showed that the diagnosis information for both residents was not verified or accurately reflected in the medical record, resulting in incomplete and inaccurate documentation for the residents.
