Infection Surveillance Log Incomplete and Hand Hygiene Not Performed Before PPE

Summary

The facility failed to maintain an accurate infection control surveillance program for January 2025. The infection prevention and control surveillance log showed surveillance was conducted only for residents who were prescribed antimicrobial medications. Residents who exhibited signs and symptoms of infection but were not prescribed antimicrobial medications were not included in the surveillance log or the monthly infection surveillance report. The facility document review also showed three residents diagnosed with influenza and prescribed antiviral medication were tested for COVID-19 but were not included in the surveillance log. During interview, the IP stated she initially completed an order listing report and then manually entered information for residents prescribed antibiotics into the surveillance log. She stated she conducted surveillance only on residents with infections who were prescribed antimicrobial medications and did not include residents who had signs and symptoms of infection but were not prescribed antimicrobial medications, including residents with influenza. The CNO later reviewed and verified these findings. The facility also failed to ensure hand hygiene was performed before donning gloves during medication administration. During observation of medication administration to two residents who were on EBP, an LVN donned gloves and a gown before entering each resident's room and administered GT medications without performing hand hygiene first. The LVN later stated that hand hygiene is intended to promote infection control and verified that she did not perform hand hygiene before donning gloves during both observed medication passes. The CNO and RN Manager were informed and acknowledged these findings.
