Care Plan Not Updated for Weight Loss and Heel Offloading

Sea Cliff Healthcare CenterHuntington Beach, California Survey Completed on 01-14-2026

Summary

The facility failed to ensure Resident 30’s care plan was revised to address the resident’s weight loss and pressure injury prevention needs. Resident 30 was admitted on 5/31/25 and had a BIMS score of 14, indicating cognitive intactness. The facility’s Nutrition Care Management policy stated that care and services should include monitoring and evaluating the resident’s response to interventions, with the care plan updated and revised as appropriate. Resident 30’s monthly weights showed 130.4 lbs. on 11/9/25 and 116.4 lbs. on 12/4/25, reflecting a 10.74% weight loss in one month. The care plan for nutrition, dated 11/19/25, included monthly weight monitoring, but the record did not show that the care plan was revised to address the actual weight loss. The monthly weight report also did not show documentation of the resident’s weight for January 2026 or whether the resident refused to be weighed. The facility also failed to revise Resident 30’s care plan to include a physician-ordered intervention for heel pressure relief. The physician ordered heels to be elevated on a pillow when the resident was in bed to offload pressure on 11/9/25. Resident 30’s care plans included a risk for skin breakdown care plan dated 5/31/25 and a resolved pressure injury to the right heel care plan dated 11/9/25, but neither was revised to include the heel-elevation intervention. The facility’s Skin Management System policy stated that a plan of care would be initiated for areas of actual skin breakdown and reviewed and revised as needed. Interviews with LVN 6 and the RD confirmed the care plan was not updated to reflect the resident’s weight loss, refusal to be weighed, or the heel-offloading order, and the DON acknowledged the findings.

Short Summary

Failure to Update Care Plans for Comfort Care and Pressure Ulcers: The facility did not revise the care plan for a resident placed on comfort care after a clinic visit showed worsening fluid retention, cough, swelling, and decreased strength; the plan omitted the no-hospitalization order, discontinuation of labs, and guidance for comfort if the resident declined. The facility also failed to update another resident’s care plan after the MDS identified four Stage II pressure ulcers, leaving only general skin-risk interventions instead of wound-specific goals and treatment measures.

D
F0657 F657: Develop the complete care plan within 7 days of the comprehensive assessment; and prepared, reviewed, and revised by a team of health professionals.
Short Summary

Care plans and related care guides were not updated for a resident with pain, a resident with recurrent falls, and a resident with severe cognitive impairment and transfer needs. One resident’s plan lacked individualized nonpharmacological pain interventions, another resident’s plan omitted a motion sensor that staff were using for fall prevention, and a third resident’s plan and Kardex incorrectly stated the resident was independent with transfers despite staff using a transfer belt and Hoyer lift with two-person assistance.

Failure to Update Care Plan With Current Diagnoses and Medication Indications
D
F0657 F657: Develop the complete care plan within 7 days of the comprehensive assessment; and prepared, reviewed, and revised by a team of health professionals.
Short Summary

A resident with a history of anemia, moderate dementia, and chronic pain had active orders for aspirin for CAD and sertraline (Zoloft) for depression and chronic pain, but the comprehensive care plan was not revised to reflect current diagnoses and medication indications. The care plan continued to reference anemia and daily aspirin for antiplatelet therapy and included a directive to administer antidepressants for chronic pain without specifying sertraline’s use for both depression and chronic pain. An MDS nurse acknowledged that the resident no longer had an active anemia diagnosis and that the care plan should have been updated to clarify the current clinical rationale for aspirin therapy and the indication for sertraline.
