Food Served at Unappetizing Temperatures
Summary
Food was not ensured to be palatable or served at an appetizing temperature for one sampled resident, R6. During interview, R6 stated food was not served hot, that the concern had been discussed at a Resident Council meeting, and that meals were still served at an unappetizing temperature, which sometimes caused R6 not to eat. R6 also stated that when nursing staff reheated food upon request, it was still not served hot or at an appetizing, palatable temperature. Resident Council meeting minutes dated 12/7/25 documented that residents reported food served cold and at an unappetizing temperature. During lunch observation in the main dining area kitchenette, the surveyor noted no steam rising from food in the steam table and did not obtain food temperatures. A thermal plate was removed from an insulated food cart, and the cart doors were open during meal service. The Dietary Manager stated the plate warmers were heated in the downstairs kitchen oven at 200 degrees F and brought up in the cart for service. During breakfast observation, the surveyor again noted no steam rising from food in the steam table. The Dietary Manager stated the steam table did not reach an appropriate temperature in the morning to heat food and maintain proper holding temperatures, that food frequently needed to be reheated prior to service, and that the far-left side well of the steam table did not work. The Dietary Manager reheated a plate of scrambled eggs, waffles, and sausage links in the kitchenette microwave, and the resulting temperatures were 145 degrees F for the eggs, 150 degrees F for the waffles, and 108 degrees for the sausage links. During lunch observation, the Regional Dietary Manager confirmed the steam table did not heat to an appropriate temperature in the morning, confirmed the left-hand side well did not work, and stated the facility was in the process of determining how to keep food warm and at an appetizing temperature. The Regional Dietary Manager also verified the facility was aware that food was not served at palatable temperatures during meal service and that residents had raised the concern during food and Resident Council meetings.
