Blood Glucose Meter Disinfection and Infection Control Policy Deficiencies

Summary

The facility failed to establish and maintain an infection prevention and control program because blood glucose meters used for resident testing were not sanitized according to the manufacturer’s instructions. During observation and interview, an LPN stated that each medication cart contained one shared blood glucose meter and that the meter was disinfected with alcohol prep pads before and after each use. Another LPN gave the same description of cleaning the shared meter with alcohol prep pads after use. No CaviWipes were observed on either medication cart, and the DON stated the nurses used alcohol wipes to sanitize the glucometers after each use. The manufacturer’s manual for the blood glucose meter stated that the meter must be cleaned and disinfected after use on each patient and that CaviWipes disinfecting towelettes were the approved product for cleaning and disinfecting the device. The manual also described a cleaning and disinfecting process that required wiping the meter with CaviWipes, allowing the surface to remain wet for 2 minutes, and then air drying. The DON stated the facility did not have CaviWipes available because they had been flushed down the toilet and caused plumbing issues, and she stated she was not aware of the manufacturer requirement to use CaviWipes to sanitize the glucometer. Thirteen residents had orders for blood glucose monitoring and were identified in the record review, including residents with diagnoses such as type 1 or type 2 diabetes mellitus, chronic HCV, chronic viral hepatitis C, severe sepsis, bacteremia, dementia, schizophrenia, and other conditions. The report also stated that the facility’s COVID-19 infection control policy and procedure provided during entrance had a revision date of August 2014, and the DON stated there must be a more updated infection control policy and procedure because it had to have been updated after 2014. The Administrator and BOM stated there was a more updated policy, but it was not located in the infection control binder.
