Summary

A resident with diagnoses including type 1 diabetes mellitus, dementia, mood disorder, and psychotic disturbance sustained multiple falls while in the facility, and the record showed no interventions developed after several of those falls. The resident reported having fallen three or four times and said someone had done exercises with her to help her get stronger after a fall. During observation, the resident was in bed with the bed not in the lowest position and the wheelchair was not next to the bed or within reach. The medical record documented repeated falls throughout the year, including falls while leaning to one side, while coming out of the bathroom, while leaving the restroom, after midnight rounds, while transferring from bed to wheelchair, while standing up from the wheelchair in the dining room, and while trying to get into bed. Some notes documented that the resident was found on the floor, while others documented unwitnessed falls or that the resident stated she had fallen. After several of these events, the notes documented only immediate assessments such as neuro checks, vital signs, call light placement, or lowering the bed, but the record review found no care plan interventions located after each of the falls described. The care plan later included interventions to assess abilities and fall risk, follow post-fall protocol, keep the bed in low position, keep the environment clear, wear non-skid socks or shoes, use therapy services, use adaptive or safety equipment, and remind the resident to ask for assistance. Staff interviews indicated the resident often tried to transfer herself, liked to close her eyes, and needed the bed in the low position, but staff were not aware of other fall-prevention interventions. The DON stated a fall checklist had only recently been started, that no checklist was available for this resident, and that staff were not following the care plan if a low-bed intervention had already been in place.
