Summary

The facility failed to establish and follow policies regarding smoking, smoking areas, and smoking safety that took into account nonsmoking residents for two residents reviewed for smoking. Resident #11 was a male admitted with diagnoses including unspecified dementia, peripheral vascular disease, and schizophrenia. His quarterly MDS showed a BIMS score of 15, indicating intact memory and the ability to make independent decisions regarding ADL care. His smoking assessments were completed on 06/03/2023, 01/11/2024, 03/08/2024, 06/10/2024, and 09/10/2024, but quarterly smoking assessments were not regularly completed. Resident #72 was a male admitted with diagnoses including ESRD, type 2 diabetes mellitus with diabetic polyneuropathy, dependence on renal dialysis, and loss of the right upper limb below the elbow and right hand at the wrist level. His quarterly MDS also showed a BIMS score of 15, indicating intact memory and the ability to make independent decisions regarding care. His smoking assessments were completed on 12/29/23, 03/08/24, and 03/06/25, but quarterly smoking assessments were not regularly completed. During interview, Resident #72 stated that because he has no hands or arms below the elbows, staff must assist him with smoking. The DON stated that a designated smoke aide is assigned to sit with residents during smoke breaks and that smoking assessments were expected on admission, quarterly, and with any change in smoking status. The Administrator stated the same expectation and said the Social Worker was expected to complete the smoking assessments.
