Failure to Follow Transfer Care Plan Resulted in Resident Injury

Summary

The facility failed to ensure a resident was free from neglect when staff did not follow the resident’s care plan during a transfer from a wheelchair to bed. Resident CR147 had a history of a right BKA, Type II DM, chronic pain syndrome, and unspecified lack of coordination. The resident’s MDS showed substantial to maximal assistance was needed for bed mobility, transfers, toileting, and movement from sitting to standing, and the care plan required transfers using a sliding board with assistance of one staff member. During the transfer that led to the deficiency, a NA moved the resident from the wheelchair to the bed without using the required sliding board. The NA documented that the resident refused the slide board and that the transfer was done by standing the resident and pivoting her to the bed while she wore her prosthetic on the right lower extremity. The resident stated that the slide board was not used and reported that the aide told her to hug her, pulled her up, and turned her while her foot did not turn. The resident reported pain in the right knee after the transfer. Clinical documentation showed the resident was assessed by the CRNP, who observed swelling of the right knee and ordered an x-ray, ice, elevation, and pain medication. The x-ray showed an acute comminuted distal femur fracture of the right leg, and the attending physician ordered transfer to the hospital. Hospital records documented that surgical repair was indicated, but the resident declined ORIF and elected to undergo an AKA, which was performed after the fracture. The facility’s investigation determined the NA failed to follow the resident’s plan of care by not using the sliding board, and the facility substantiated neglect related to the event.
