Summary

The facility failed to ensure that food stored in the kitchen was labeled, dated, and secured in accordance with its policy and professional standards. The facility policy dated January 19, 2026 stated that any food that has been opened must be labeled, dated, and secured in such a way that the food item is airtight, and that proper labeling and dating helps ensure foods are stored, rotated, and utilized to minimize waste and discard items past their due date. During observations on January 20, 2026, surveyors found multiple opened food items in the kitchen cooler without an open date, including cabbage with carrots, bacon, cinnamon rolls, mozzarella cheese, a jar containing pickles and tomatoes, broccoli salad, beefaroni, and wrapped cookies. In the walk freezer, opened items including stuffing and muffins were not dated, and beef fritters and sugar cookies were not labeled, dated, or secured. In the dry storage room, opened items including cereal, brownie mix, spaghetti, and dry milk were not dated, and corn flakes, raisin bran, coconut shavings, and Hershey [NAME] were expired, outdated, or unsecured. The Dietary Manager confirmed that all food items in the kitchen should be labeled, dated, and secured, and they were not.
