Summary

The facility failed to follow its pre-approved planned menu and failed to inform residents of menu changes. The posted lunch menu for Wednesday, January 21, 2026, listed fried chicken, macaroni and cheese, sliced carrots, cornbread with margarine, marble cake with white frosting, and a beverage, with an alternate meal of a hot dog with chili and cheese on a bun, potato wedges, creamy coleslaw, and a beverage. However, observation of a test tray at 11:35 a.m. showed baked chicken instead of fried chicken and no cornbread was provided. The alternate meal served was meatballs and gravy with mashed potatoes instead of the posted hot dog with chili and cheese on a bun and potato wedges. During interview at 12:25 p.m., the Dietary Director and District Manager confirmed that the menu items served did not match what was posted, that cornbread had been changed to a dinner roll, and that the changes were not posted for residents to see or communicated to residents verbally.
