Summary

The facility failed to ensure appropriate prevention and management of a pressure injury for one resident with chronic kidney disease, severe cognitive impairment, limited mobility, incontinence, and a history of pressure injuries. The resident had an unstageable coccyx pressure ulcer, and the clinical record showed the resident required staff assistance with bed mobility and transfers. A quarterly MDS dated January 10, 2026, identified the resident as severely cognitively impaired with a BIMS score of 1 and at very high risk for pressure injuries due to being chairfast, having limited mobility, and being constantly moist from urinary incontinence. Investigative documentation dated December 28, 2025, showed staff identified a coccyx pressure ulcer measuring 1 cm by 0.5 cm, but the investigation did not document the wound depth, determine the stage, describe the condition of the coccyx area before the wound was found, or describe the surrounding skin. The report stated the resident had a previously healed pressure injury in the same location that reopened after healing on November 24, 2025, and identified immobility, incontinence, and fragile skin as possible contributing factors. However, the investigation did not determine whether the resident's preventive interventions were consistently implemented or whether a change in condition or care contributed to the reopening of the wound. The care plan included repositioning interventions while in bed and in a chair, and staff task documentation for February 2026 indicated the resident was to be repositioned every hour. A review of staff task documentation from February 1 through February 26, 2026, showed the repositioning documentation was blank and did not show that the required interventions were completed. The resident also used a Med-Aire Melody Alternating Pressure Low Air Loss Mattress, but observations on February 26 and February 27 showed the mattress was set to 150 pounds despite the resident weighing 98.5 pounds. A later wound assessment documented the coccyx pressure ulcer as 2.0 cm by 1.0 cm by 0.20 cm and unstageable, and the DON was unable to provide evidence that the repositioning schedule had been followed or that the incorrect mattress setting had been identified and corrected.
