Summary

The facility failed to provide nursing services consistent with professional standards of quality when licensed nurses repeatedly administered Midodrine to Resident 7 outside the physician-ordered hold parameter. Resident 7 had diagnoses including congestive heart failure and chronic atrial fibrillation. A physician order dated January 9, 2026 directed staff to give Midodrine 5 mg by mouth three times daily for orthostatic hypotension and to hold the medication if systolic blood pressure was greater than 110 mm/Hg. Review of the January 2026 MAR showed Midodrine was administered 31 times when the documented systolic blood pressure exceeded 110 mm/Hg. The blood pressure readings recorded at the time of administration included multiple values above the ordered limit, such as 122/66, 146/96, 125/72, 126/80, 128/68, 137/78, 154/100, 141/70, 135/78, 141/98, and 122/80 mm/Hg. Review of the February 2026 MAR showed eight additional administrations when systolic blood pressure was above the ordered parameter, including readings such as 145/96, 124/73, 119/69, 129/74, 122/84, and 121/74 mm/Hg. The facility also failed to ensure consistent implementation of a physician-ordered lateral support for Resident 4. Resident 4 had diagnoses including right femur fracture, lumbar disc degeneration, and muscle weakness. A physician order dated October 20, 2025 directed staff to provide a lateral support to the right side of the resident's wheelchair for positioning and stability. During observation, Resident 4 was seated in the wheelchair leaning significantly to the right, and the ordered lateral support was not attached; it was found in a laundry basket on a chair in the resident's room. A later observation with an LPN confirmed the lateral support was still not in place.
