Summary

The facility failed to provide effective pain management and failed to implement, monitor, and revise pain interventions for two residents with documented pain needs. The facility’s Pain Management Protocol stated that residents’ pain would be assessed to maintain their highest practicable level of well-being, that treatment effectiveness would be monitored and adjusted, and that both medication and non-medication techniques would be used, with documentation of effectiveness or need to adjust treatment when medication-related interventions were required. Resident 11 was admitted with chronic pain and a Stage IV sacral pressure ulcer. The resident was cognitively intact and reported that dressing changes to the sacral wound were very painful, including removal and replacement of wound packing. The care plan addressing pain related to skin integrity did not identify or consider pain during wound treatment procedures or include interventions to reduce procedural pain. The physician orders and treatment record did not show specific pain medication orders coordinated with the sacral dressing changes, and there was no conclusive evidence that pain medication was offered prior to treatment during the wound care episodes reviewed. The DON was unable to provide evidence that the resident’s procedural pain during wound treatments had been addressed or that medication administration had been coordinated with treatment to reduce pain. Resident 4 had diagnoses including osteoarthritis, cervical disc disorder with radiculopathy, and Hodgkin’s lymphoma, and was cognitively intact with documented pain that was almost constant and increasingly interfered with sleep and daily activities. The resident reported chronic pain, stated that oxycodone had previously been ordered PRN but was changed to once-daily bedtime dosing, and described severe pain after an off-site appointment when Tylenol did not relieve the pain. Pain assessments showed worsening pain over time, but the clinical record did not show that the pain regimen was reassessed or revised, and there was no evidence that the attending physician was timely notified about the increased pain complaints or the resident’s report that current interventions were not effective. The DON confirmed that licensed nursing staff failed to timely notify the physician and failed to obtain new or revised orders to effectively manage the resident’s pain.
