Summary

The facility failed to follow care plan interventions intended to ensure safe transfers and prevent falls for three residents. The report states that the facility did not follow the resident-specific interventions for safe toileting transfers, fall precautions, and bed positioning that were documented in the care plans and assessments reviewed by surveyors. For one resident, the care plan required two staff members for toileting and transfers, but a CNA attempted a one-person transfer from a wheelchair to the toilet. For a third resident, the care plan directed staff to keep the bed in the lowest position outside of care. Survey observations found the bed repeatedly positioned between knee and waist height rather than in the lowest position. Staff interviewed were unaware of the fall precautions or stated the resident was not a fall risk, while the MDS and care plan identified the resident as a moderate fall risk and dependent on staff for mobility and transfers.
