Resident 82 was admitted with other idiopathic peripheral autonomic neuropathy and was discharged, but the discharge assessment was not evidenced as transmitted. Resident 83 was admitted with stroke and dementia and was discharged, but the discharge assessment was not evidenced as transmitted. The ADM stated he was not aware the Discharge MDS had not been transmitted until the day of the interview and confirmed the 14-day submission timeframe.

The MDS Coordinator stated the missing entry assessments should have been done and that the facility was behind on MDS assessments.
