Summary

Food served to residents from the kitchen was not palatable. The lunch menu for 08/11/25 listed beef teriyaki, white rice, sugar snapped peas, a choice of roll, and lemon bars, but residents in the dining room were served a beef steak patty with white gravy instead of beef teriyaki. At 12:47 p.m., a resident not included in the survey sample was overheard saying, "This is one of those days I wish I had money to Door Dash food." At 12:52 p.m., Resident #28 was observed having difficulty cutting the beef patty and pushed the plate away. At 12:59 p.m., Resident #4 told their one-on-one sitter they could not cut the meat, and the sitter was then observed cutting Resident #4's beef patty. At 1:04 p.m., Resident #8 had eaten 25% of the lunch plate and later stated the meal was "horrible" and "Awful," adding, "I couldn't even chew this mystery meat." When the surveyor attempted to cut the beef patty at 1:23 p.m., it took one solid minute using all hand strength to cut into it; the patty did not taste fresh, was not easily chewed, was bland, and had burned edges. At 1:24 p.m., the dietary manager was asked to cut a piece of the beef patty and tried three times before stating, "I can't cut it." The dietary manager also stated they normally taste the food before serving it but did not do so that day, and said they would not expect a resident to be able to cut the beef patty on their own.
