Summary

The facility failed to ensure that oxygen was administered according to physician orders for Resident #45 and failed to ensure that Resident #24 had an order for oxygen use. Resident #45 had diagnoses including unspecified COPD and unspecified heart failure, and a physician order dated 04/01/25 directed oxygen at 2 liters via nasal cannula every shift. However, the resident was observed on 4.5 liters of oxygen on 08/11/25 and again on 08/12/25. The resident stated they were on 2 liters, and LPN #1 later stated the resident was supposed to be on 2 liters but the oxygen rate was set to 4 liters. LPN #1 reduced the oxygen rate to 2 liters after identifying the discrepancy. Resident #24, who had diagnoses including COPD and unspecified heart failure and was cognitively intact, was observed on 3.5 liters of oxygen via nasal cannula. The resident stated they had been on 3 liters of oxygen since 12/2024 and that staff only checked their oxygen when they asked. LPN #1 stated the resident was supposed to be on 2 liters of oxygen, observed the concentrator set at 3 liters, and then stated the resident did not have an order for oxygen. The DON stated there should be an order for the use of oxygen.
