The resident received lorazepam 0.5 mg every four hours PRN agitation under a hospice physician order with no end date. The MAR showed the medication was administered repeatedly in July and August, and the medical record did not contain documentation from a physician or prescriber giving a rationale for extended use or an end date. The ADON stated the facility policy was a 14-day stop date for PRN psychotropics and acknowledged there was no stop date for the resident’s lorazepam. The facility also failed to discontinue psychotropic medications per orders for another resident with recurrent depressive disorder, PTSD, and generalized anxiety disorder. The resident had orders for doxepin 6 mg at bedtime and later doxepin 10 mg at bedtime, with a physician order stating to stop the 6 mg dose once the 10 mg was available. However, the MAR still showed both doses being administered, and a refill request for the 6 mg dose was signed by an LPN. A pharmacy recommendation noted the duplicate therapy and asked whether the resident was supposed to be on both orders, but the document did not show DON or medical director review. The ADON later acknowledged the 6 mg dose was supposed to be stopped.

The resident’s RP stated the resident’s sign of restlessness was isolating himself and reported that the resident texted her about having anxiety from 10:30 AM until 7 PM on 6/30/2026. The DON reviewed the MAR and stated it was not accurate because staff had different ideas of what restlessness was, and stated the resident should have the correct manifestation of anxiety reflected in the physician order and MAR. The facility policy stated psychotropic medications are to be used only when a practitioner determines the medication is appropriate to treat the resident’s specific, diagnosed, and documented condition.
