Summary

Weekly skin assessments were not completed for a resident with diagnoses including cerebrovascular infarction, traumatic brain injury, hemiplegia, chronic pain, and neuromuscular dysfunction of the bladder. During observation, the resident was seen being assisted into bed after a shower and was noted to have an open area and multiple scabbed areas of various sizes on the left buttocks. The physician’s order required weekly skin assessments every Monday on the day shift, but the only skin assessment found was dated 12/11/25 and showed no skin problems; no additional skin assessments were documented after that date. The resident’s quarterly assessment showed a BIMS score of 6, indicating moderate cognitive deficit, and the DON stated the wound care nurse was responsible for completing weekly skin assessments. Wound treatment was not completed as ordered for another resident with a right anterior ankle/lower leg wound. The physician’s order required cleansing the wound with normal saline, applying Medi-honey, covering with an abdominal pad, wrapping with Kerlix, and securing with tape, with treatment daily and as needed if soiled until the wound care consult was complete. A wound care visit report later changed the treatment to wound cleanser, honey gel, silver alginate, and a super absorbent dressing on Monday, Wednesday, and Friday, but the order in the chart was not updated. During observation, an LPN performed the wound treatment without using silver alginate as ordered, and later stated the wound care provider was responsible for the resident’s wound orders and that the resident’s orders had not been updated when changed.
