Lack of Privacy for Resident Telephone Calls

Bellbrook Health And RehabBellbrook, Ohio Survey Completed on 01-15-2026

Summary

The facility failed to provide privacy for telephone calls for one resident reviewed for telephone use. Resident #50 was admitted with diagnoses including hemiplegia and hemiparesis following cerebral infarction affecting the right dominant side, anxiety disorder, post-traumatic stress disorder, and depression. The admission data collection form documented the resident as alert and oriented times four. During observation, Resident #50 was seen at the 300-Hall nursing station using the desk telephone for a personal call while multiple residents were in the hallway, and later was again observed at the nursing station desk talking on the telephone with another resident sitting directly next to her. Staff interviews confirmed the lack of private telephone access on the 300-Hall. The BOM stated residents were usually given the cordless telephone from the front desk, but Resident #50 was using the regular desk telephone. An LPN stated Resident #50 asked to call her father and was handed the desk telephone, adding that it was the only telephone she had to use and there was no private telephone. Resident #50 stated she was not offered any private area or private telephone for personal calls and preferred a private area. An RN and the Maintenance Director both stated the cordless telephone at the main nursing station was used for residents to make private calls, but it did not work on the 300-Hall. The resident admission packet stated residents have the right to reasonable access to a telephone and a place where calls can be made without being overheard.

Failure to Ensure Timely Weekend Mail Delivery to Residents
C
F0576 F576: Ensure residents have reasonable access to and privacy in their use of communication methods.
Short Summary

The facility failed to ensure residents received their mail in a timely manner, particularly mail delivered on Saturdays. During a council meeting, several residents reported that Saturday mail was not brought to them until Monday. The new Activity Director did not work weekends and was unfamiliar with the weekend mail process. The weekday receptionist stated that the weekend receptionist had no key to the outdoor mailbox, so mail was not retrieved on Saturdays unless the Business Office Manager was present. The Business Office Manager confirmed that when she was absent on weekends, no one else could access or deliver residents’ mail, and that after the prior Activity Director left, there was no designated staff to deliver weekend mail, resulting in periods when residents did not receive their mail on the day it arrived.

Observation showed no accessible phones available, and the Administrator and RDO confirmed the facility’s resident-use phones were not accessible to the resident, so staff had been using personal cell phones to help him make calls.

Failure to Ensure Resident Access to Private Telephone Communication
D
F0576 F576: Ensure residents have reasonable access to and privacy in their use of communication methods.
Short Summary

A resident with multiple chronic conditions, who relied on phone contact with a seriously ill significant other, lost access to private communication after her personal cell phone was broken and sent out for repair. Facility policy guarantees residents access to a telephone and private communication, but there was no cordless phone available on the relevant hall, and staff, including the DON, SSD, and Administrator, confirmed there was no convenient method for residents on that hall to make private calls without arranging to use a staff office. This resulted in the resident having no readily available, private telephone option.
