The facility failed to post, on a daily basis at the beginning of each shift, the current resident census and the total number and actual hours worked by licensed and unlicensed nursing staff directly responsible for resident care in a prominent location readily accessible to residents and visitors for 5 of 5 days. Survey observations found the current daily resident census, staffing hours, and nurse staffing schedules posted on the Unit 1 nursing office door and window, which was not easily accessible to all visitors or residents. The facility policy, dated 01/16/2024, stated that nurse staffing information would be posted on the first floor next to the nursing office window at the beginning of each shift and would be legible and posted in a prominent place accessible to residents and visitors. During multiple observations, staffing information was seen on the first floor nursing office door and window across from the Unit 1 elevator, and a sign in the main lobby above the visitors' sign-in sheet was about 5 feet from the ground on a pillar. During interview, the DON stated the staffing numbers were posted on the glass window of the nursing office on the first floor, there was no staffing sheet posted on the second floor at the main entrance, and the sign was not easily seen from wheelchair height.

During observations on 6/1/2026, staffing information dated 6/1/2026 was posted at the main entrance, but the census listed as 103 did not match the Resident Listing Report census of 102. Later that same day, staffing information dated 6/2/2026 was observed posted, and on 6/2/2026 staffing information dated 6/3/2026 was posted at the main entrance. On 6/3/2026, staffing information dated 6/4/2026 was posted at the 100 Hall nursing station near the off-hours facility entrance. During interview, the DON confirmed the staffing information posted was dated 6/4/2026 and stated the scheduler would prepare the staffing information sheet before leaving and corrections would be made the next day. The DON also stated the staffing sheet should not be posted in the front and should be placed behind the current day until the new shift starts. Administration stated the staff coordinator should not post the staffing information the day before and should post the current staffing information for the day. The Staffing Coordinator stated she had been putting the staffing information out before leaving at the end of the day for the following day and did not notice the census was incorrect.

The facility policy, Nurse Staffing Posted Information dated 02/01/2026, stated that nurse staffing information was to be made readily available in a readable format to residents, staff, and visitors at any given time.

During interview, NA-A verified the posting was dated 5/26/26 and stated it was important for families and visitors to see how many staff were working. SC-B stated she changed the nurse staffing hours on Thursdays for the coming week but did not update the posted sheets, and instead made the changes in the computer after they occurred when she returned on Mondays. A review of the next week's schedules showed the census as 24, and one schedule dated 5/30/26 had zeros for all staff listed with a census of 24. The DON verified the nurse staff posting should be posted daily so anyone could see how many residents were in the facility and how many staff were working, and stated it was important so families could have confidence there was enough staff to take care of their family members.
