Summary

The facility did not ensure Elevator #2 was maintained in safe working condition. During the recertification survey, surveyors observed that Elevator #2 was broken and out of service for over 10 months, while a security staff member was positioned inside the elevator to move passengers from floor to floor and unit staff were paging front door security to send the elevator to various units when needed. The building is six floors with three elevators, including two passenger elevators and one service elevator used primarily for goods, equipment, and staff. During the Resident Council meeting, six of eight residents in attendance stated they had been concerned about the broken elevator for over 10 months and said it prevented them from attending programs in the facility. One resident reported being stuck inside Elevator #2 and said it took more than 30 minutes to get them out. The Director of Recreation acknowledged awareness of the broken elevator and stated residents experienced delays moving around the facility and had to wait or page for the elevator where needed. The Director of Maintenance Services confirmed Elevator #2 had been damaged for over 10 months, said complaints had been received from residents, family members, and staff, and stated the needed part was hard to get and the repair timeline was unknown. The Administrator confirmed Elevator #2 had been out of service since November 2024 and stated it was under repair.
