The facility policy stated the assessment was to address resident population, staff competencies, equipment, services, and personnel, including contract staff. However, the August 2025 Facility Assessment did not include ventilator-dependent residents in the resident population or identify the staff competencies, ventilator equipment, associated services, physical plant operations, or all personnel needed to care for that population, including respiratory therapist coverage. The Resident Matrix for Unit 3 documented 11 ventilator residents, and the CMS-671 documented 20 ventilator/respiratory care beds. The Employee Contact List showed 5 respiratory therapists, and the Director of Respiratory Services stated Unit 3 currently housed 11 ventilator-dependent residents and that they were working as a full-time RT on Unit 3. The Facility Assessment also listed onsite hemodialysis as a service provided even though the facility did not have a certified dialysis unit. The Facility Administrator stated the assessment was an overview of residents and care provided and acknowledged that ventilator services were not listed as a service provided and that hemodialysis had been erroneously listed. The assessment documented a total bed capacity of 228 and listed services including hospice, palliative care, respite care, LTC, subacute rehabilitation, hemodialysis, bariatric, dementia, traumatic brain injury, PASSR Level Two, cancer treatments, and substance use disorders, but it did not accurately reflect the ventilator service population or the facility's actual dialysis capability.
