Missing Annual CNA Performance Reviews and Related In-Service Education
Summary
The facility did not ensure that a performance review of every nurse aide was completed at least once every 12 months, and that regular in-service education was based on the outcome of those reviews. During the recertification survey, review of the Sufficient and Competent Nursing Staffing task found that six of six Certified Nursing Assistant personnel files contained no evidence of yearly performance evaluations and no in-services based on the results of those evaluations. The facility policy titled "Staffing, Hiring, Application Process and Training," last reviewed 02/06/2024, stated annual performance reviews would be completed for each staff member and specific in-service education would be completed based on the outcome of the annual performance review. Review of the personnel records for CNA #8, #9, #10, #11, #12, and #13 found no documented annual performance reviews and no in-service education tied to those reviews. The HR Director stated it was their responsibility and the DON's responsibility to ensure yearly performance reviews were completed, but they had not reviewed older employee files for compliance. The DON stated performance reviews were important and that evaluations were forwarded from HR for nursing supervisors to complete, but they had not received any request to complete performance evaluations. The Facility Administrator stated performance evaluations should have been performed yearly and as needed and took full accountability for them not being completed.
