Summary

The facility failed to ensure that a resident was cared for in a manner that maintained dignity when Resident #12’s foley catheter drainage bag and tubing were left uncovered. Resident #12 was admitted with diagnoses including obstructive uropathy, seizure disorder, and acute kidney failure, and the admission MDS documented severely impaired cognitive skills for daily decision making, extensive assistance needed for bed mobility, transfers, dressing, toilet use, and personal hygiene, and the presence of an indwelling catheter. The physician order directed foley catheter care every shift and monitoring/documentation of urine output every shift for neuromuscular dysfunction of the bladder. During multiple observations, Resident #12 was lying in bed with amber urine draining into the foley catheter bag, and the bag and tubing were visible from the hallway because they were not contained in a privacy bag. Staff interviews confirmed the omission: an RN stated they were not aware the drainage bag had no cover and said it should have been covered, an LPN stated the bag must be covered at all times because it preserves dignity, and the DON stated they were not aware the foley had no privacy cover and that it was the responsibility of unit staff, especially the nurse, to ensure the privacy cover was in place.
