Summary

Provide or obtain dental services for each resident was not ensured for one resident who had been admitted to the facility in June 2024 and was never seen by a Dentist. The resident had diagnoses including hypertension and type 2 diabetes mellitus. The 5-Day MDS documented a BIMS score of 15, indicating intact cognitive skills for daily decision making, and noted no broken or loose fitting full or partial denture and no mouth or facial pain, discomfort, or difficulty with chewing. During interview, the resident stated they had dentures in the community, but did not have their upper and lower dentures when they came to the facility, had never been seen by a Dentist since admission, and wanted their dentures replaced. Record review found no documented evidence that the resident was ever seen by a Dentist. A physician's order dated 01/16/2026 documented a dental consultation in house as needed. Staff interviews indicated the resident was originally admitted for short-term rehabilitation and later became a long-term care resident, at which time they should have been seen by a Dentist. The Assistant Director of Nursing Services stated they did not know why the resident was never seen by a Dentist, Registered Nurse #2 stated all consultations, including dental consultations, require a physician's order, the Medical Director stated a physician's order is required for a dental consult and that this should have been documented in the facility's Dental Policy, and the DON stated the resident was never seen by a Dentist and it was an oversight.
