Summary

The facility failed to follow the planned menu to meet the nutritional needs and preferences of all 109 residents listed on the census when the posted lunch menu was not served as written. During an observation of lunch service in the main dining room at 11:40 a.m. on 01/04/26, residents were served ice cream for dessert. Review of the written lunch menu for that date showed that the planned meal items were pizza, broccoli, and strawberry streusel, indicating that strawberry streusel, not ice cream, was the scheduled dessert. In an interview at 12:16 p.m. on the same day, the Dietary Manager stated that an employee had called off for a shift, requiring her to cover the shift, and that she arrived late to the facility and did not have time to prepare the strawberry streusel, resulting in the substitution of ice cream for the planned dessert.
