Summary

The facility failed to ensure abnormal involuntary movement scale (AIMS) evaluations were accurate and completed timely for three residents receiving antipsychotic medications. For one resident with bipolar disorder, major depressive disorder, and anxiety disorder, the record showed olanzapine was ordered, but the April AIMS had no documentation in the examination procedure section for tardive dyskinesia, and the November AIMS was not found in the EMR. The DON stated the November AIMS was not triggered because the form was marked that the resident was not administered an antipsychotic medication, and the DON later confirmed the April AIMS was not complete. For a second resident with bipolar disorder, anxiety disorder, major depressive disorder, and dementia, the record showed quetiapine was ordered twice daily and at bedtime, and an April 2024 AIMS documented a score of 21 with tardive dyskinesia. The resident was also prescribed Austedo for TD, but the care plan did not include a focus, goal, or interventions for its use. A September 2025 AIMS documented no TD symptoms and a score of 0, and the RN who completed it stated he had not received training on performing AIMS assessments and acknowledged one section documenting that TD movements did not disappear in sleep was wrong. For a third resident with major depressive disorder, altered mental status, catatonic disorder, generalized anxiety disorder, bipolar disorder, and PTSD, the record showed lurasidone was ordered daily and the MDS indicated daily antipsychotic use. However, the July and October quarterly evaluations did not document monitoring for tardive dyskinesia, both were marked as if the resident did not receive an antipsychotic medication, and the AIMS sections were left blank. The ADON confirmed the AIMS evaluations should have been completed for this resident because the resident was receiving Latuda.
