Summary

The facility failed to ensure the Minimum Data Set (MDS) accurately reflected the presence of a stage III pressure ulcer for one resident. Review of the resident’s EMR showed diagnoses including dementia, aphasia, depression, anxiety, and chronic pain. The quarterly MDS with an ARD of 07/31/25 indicated no current pressure ulcers, even though the resident’s care plan dated 05/23/25 documented a stage 3 pressure ulcer on the sacrum related to impaired mobility and incontinence, with interventions including heel booties and enhanced barrier protection for infection prevention. Record review also showed wound consultation reports dated 06/20/25, 07/18/25, and 08/01/25 documenting that the resident’s healed sacral wound had re-opened as a stage III pressure ulcer and continued to require treatment and offloading instructions. The TAR for July 2025 included physician orders for sacral wound care, including cleansing, collagen or prisma/collagen application, zinc oxide to the periwound, protective dressing, repositioning side to side, and monitoring for changes, with the treatments documented as completed. During interview, the MDS Coordinator stated she missed coding the sacral pressure ulcer because she believed there was no documentation showing it re-opened after healing, but acknowledged the paperwork showed the ulcer during the MDS lookback period and should have been coded.
