RN MDS A stated that office notes are obtained after provider appointments and diagnoses are added to the MDS and face sheet, but she was not aware that the new diagnoses would trigger a new PASARR or that they should have been care planned. SS-B confirmed a new PASARR should have been done and the diagnoses should have been added to the care plan, and LPN C stated that only new orders or follow-up appointments returned from appointments and that leadership could add diagnoses or objectives to the care plan.
