Cold Meals Served During Tray Delivery

Summary

Food and drink were not maintained at a palatable, attractive, and safe temperature during meal service. The facility’s On Tray Dietary Policies and Procedures required food temperatures to be taken and logged when cooked and again before meal service, with hot foods served at 135 degrees F or higher and cold foods/beverages at 41 degrees F or lower. During observations on the 300 and 400 halls, meal trays were delivered in large metal carts and passed to residents in dining areas and rooms, but the last trays were still being served well after the carts arrived. Surveyors measured biscuits at 102 degrees F, green beans at 106 degrees F, beef stew at 100 degrees F, eggs at 115 degrees F, and milk at 50 to 54 degrees F. Five cognitively intact residents reported that meals were cold and unappetizing. One resident with paraplegia, quadriplegia, kidney calculus, cachexia, chronic pain syndrome, and mild protein-calorie malnutrition said food was always cold and tasted very nasty. Another resident with hemiplegia, diabetes, anemia, dysphagia, and muscle weakness said residents waited up to an hour for food and it arrived cold. Other residents with chronic kidney disease, depression, anxiety, heart failure, high blood pressure, liver disease, heart disease, and major depressive disorder also reported cold meals, including in-room dining. Staff interviews indicated the warming station had recently stopped being used, the steam table had been discontinued while the kitchen was being organized, and cold drinks were being served with the trays rather than kept in ice buckets.
