The resident was observed using regular utensils and a regular plate. A CNA stated the meal ticket showed the resident was supposed to have padded foam utensils, but the divided plate was not listed. The Dietary Manager stated the facility ran out of padded utensils when staff kept them in rooms and did not return them to be cleaned and reused, and that the divided plate was not on the meal ticket because the order had not been received to update the ticket. The Director of Rehab stated the resident was on therapy and the order came from that department to increase the resident's independence with eating.

At the time of the observation, the resident stated he frequently did not get the rocker knife or high sided plate and that it made it harder for him to eat his meals. The facility policy on Adaptive Feeding Equipment stated dietary must be notified of adaptive equipment needs and ensure the equipment is placed on the tray at each meal.
