Summary

The facility failed to ensure that necessary vision services were provided for one resident who was admitted with diagnoses including diabetes mellitus with neuropathy, ketoacidosis, foot ulcer, and hyperglycemia. The resident was cognitively intact with a BIMS score of 15 out of 15 and had signed a consent/request for eye health services. During interview, the resident reported worsening blurry vision, stated the left eye was worse than the right, and said the eye doctor had been promised but had not seen the resident for months despite repeated requests. The record showed a physician order for consults including optometry/ophthalmology, and a social service note documented the resident’s request to be seen by the eye doctor, with nursing made aware.
