Infection Control and PPE Failures During Resident Care

Summary

The facility failed to maintain an infection control program when a RN completed a medication pass and then immediately began preparing medications for the next resident without washing hands or using hand sanitizer after handling the previous resident’s cup and garbage. The RN confirmed this observation during interview. The facility also failed to ensure equipment used in an Enhanced Respiratory Precautions room was cleaned after use. A mobile vital signs monitor was used in the room, moved outside the room without being sanitized, and later relocated to another area while still uncleaned and available for use. The Infection Preventionist confirmed the monitor had been used in the room, had been out of sight of the staff member who initially used it, and had remained uncleaned for at least 40 minutes. The facility further failed to use the required PPE in rooms designated for Enhanced Barrier Precautions. In one room, a CNA entered while wearing a surgical mask, gown, and gloves but not an N95 respirator or eye protection, despite a posted sign requiring gown, N95 or higher respirator, eye protection, and gloves. In another room, a charge nurse performed wound care on a resident with pressure ulcers to the heel and buttocks without wearing a gown, even though an EBP sign indicated mask, gloves, and gown were required for direct care. The DON stated the facility had misread its EBP policy and believed EBP applied only to chronic wounds, and that the resident was not on EBP because the wounds developed after admission.
