Summary

The facility failed to conduct annual performance reviews for two of five direct care staff whose records were reviewed. The facility had a census of 33 residents at the time of survey. Review of employment records showed that one CNA, hired on 11/01/22, did not have an up-to-date annual performance evaluation, with the last evaluation dated 10/03/24.
