Summary

The facility failed to serve food under sanitary conditions to prevent foodborne illness during one observed meal service. During lunch service, Staff D, a cook, washed hands, applied gloves, and began serving food, but then used the same gloved hands to touch spatulas, scoop handles, the counter, and food containers before touching a peanut butter and jelly sandwich and placing it on a plate for a resident. Staff D then removed the gloves, washed hands, and put on new gloves, but again touched the glove box, utensils, a ladle, scoop handles, and the countertop before touching another sandwich and placing it on a plate for a resident without completing hand hygiene or changing gloves. The Dietary Manager stated staff were expected to use gloves one time when touching food and not touch other items before touching food items. The facility policy on Bare Hand Contact with Food and Use of Plastic Gloves stated single-use gloves are to be used for only one task, such as working with ready-to-eat food, and that gloves must be changed anytime a staff member touches a contaminated surface.
