Summary

The facility failed to ensure meals were served with proper hand hygiene, coffee was covered when transported in hallways, food trays were air dried before storage, and the kitchen environment was clean, including ceiling vents and the soiled dish area. These findings were identified during observations of the kitchen and meal service areas and were reported as affecting 102 of 109 residents residing at the facility. During kitchen observation, a dietary staff member was seen removing clean, wet food trays from a dishwashing rack and stacking them on a cart, while dust was noted on ceiling vents above the food preparation and food service areas. During lunch service on the Cambridge Unit, a coffee pitcher without a lid was observed on top of the food cart; nursing staff reached up, removed the pitcher, poured coffee for residents, and returned it to the cart while it remained open to air. Staff stated the pitchers came from the kitchen with small plastic lids that were not secured and could fly off when the cart was moved. Later observations showed continued dust buildup on ceiling vents, splatters of dried food substances on the walls in the soiled dish area, and a buildup of black substance on the caulked area and black spatters on the tiled wall behind the garbage disposal area. Dietary staff member 9 was also observed placing food trays on the serving line with water drops on them, then adjusting his beard net strap and pulling up his hood before drying the trays with a cloth without performing hand hygiene or changing gloves.
