The facility failed to identify and monitor the use of lap trays for 2 residents reviewed for restraints. The nurse stated the resident could sometimes remove the lap tray but other times could not. The ADON stated the facility had not completed a restraint assessment, had not care planned the lap tray, and had not been documenting its use.
