Missing Stop Dates for PRN Psychotropic Medications

Review of the EMR showed an order for Ativan oral tablet 0.5 mg with a start date of 12/22/2025 and an order for Haloperidol Lactate oral concentrate 2 mg/ml to be given every 6 hours as needed for agitation, nausea, or vomiting with the same start date. The consultant pharmacist had previously notified the physician that PRN antipsychotics are limited to a hard 14-day stop unless the physician completes a physical assessment and rewrites the order, and the physician agreed and continued the Haldol order for 14 days. During interviews, the MDS Coordinator confirmed the Haldol PRN order had no end date and stated the hospice provider sends a list of medications that hospice residents will take and the facility writes the order as received from hospice. The DON stated she reviewed the pharmacy report for recommendations to add stop dates and confirmed Haldol requires a 14-day automatic stop date, while nursing staff had been educated about stop dates for PRN psychotropic medications. An LPN and an RN both reviewed the orders and confirmed that Haldol and Ativan had no end date, and the RN stated Ativan, Morphine, and Haldol were part of a comfort pack for hospice residents. The physician stated it was his understanding that pharmacy sends an automatic stop date for PRN psychotropic medications.

R10 received brexpiprazole 1.5 mg twice daily for agitation related to dementia and haloperidol 2 mg daily for delusions related to unspecified dementia, but the EHR did not show evidence of side effect monitoring for these antipsychotics. R41 had severe cognitive impairment and diagnoses including non-traumatic brain dysfunction, hypertension, PVD, hyperlipidemia, arthritis, Alzheimer's disease, stroke, and non-Alzheimer's dementia. R41 received risperidone 0.25 mg three times daily for hallucinations, and the EHR did not show evidence of side effect monitoring. R91 had severe cognitive impairment and schizophrenia and received olanzapine and risperidone for paranoid schizophrenia, but the EHR did not show evidence of side effect monitoring. The record also showed failures to obtain and document orthostatic blood pressures as ordered for R10, R41, and R91. For R10, monthly orthostatic blood pressure documentation contained identical readings across lying, sitting, and standing positions on multiple dates. For R41, some monthly orthostatic blood pressure checks were documented as refused, with no evidence the monitoring was attempted again later, and other entries showed identical readings in all positions. For R91, one monthly entry lacked standing blood pressure and pulse documentation, and another showed identical readings in all positions. Staff interviews confirmed orthostatic blood pressures should be completed monthly in lying, sitting, and standing positions, that identical readings in all positions were not expected, and that side effect monitoring for psychotropic medications was not yet in place.
PRN Lorazepam Orders Lacked Required Stop Dates

The DON also stated the resident should have indications for antipsychotic use and that this did not meet expectation.
