Failure to Follow EBP and PICC Dressing Change Requirements

Summary

The facility failed to follow its infection control standards for Resident 71, who had an indwelling urinary catheter and was on enhanced barrier precautions due to the presence of a urinary catheter. The resident’s record showed severe cognitive impairment and total dependence on staff for activities of daily living. During observation, CNA 1 provided morning care without wearing a gown, even though signage outside the room indicated staff were to wear gloves and gowns for toileting, changing briefs, ADL care, device care, and other high-contact activities. When asked, CNA 1 stated a gown should have been worn and said the resident was under EBP, while also stating the gown helps prevent spread of infection. During a separate observation, LVN 5 provided catheter care to the resident without wearing a gown, and stated a gown should have been worn to prevent spreading infectious organisms. The facility also failed to change Resident 94’s PICC dressing within the required 7-day interval. Resident 94 was admitted with diagnoses including infection following a procedure, sternotomy surgical site, surgical site aftercare following surgery on the circulatory system, anemia, hypertension, muscle weakness, difficulty walking, and GERD. During a facility tour, the resident was observed with a PICC dressing on the left upper arm dated as changed on 2/8/2026. The physician order required IV central line dressing changes every 7 days and as needed. RN2 stated PICC dressings are changed weekly and as needed if soiled, and that the purpose is to prevent infection and assess the skin. RN2 stated she was unaware the dressing had not been changed for 8 days. The DON stated PICC line changes are done every 7 days and as needed if soiled to prevent infection at the catheter site and that infection could result in sickness, unnecessary hospitalization, and extended antibiotic therapy. The facility’s policies stated enhanced barrier precautions require gowns for high-contact care activities including device care such as urinary catheters and PICC lines, and that IV dressing changes are to be done at least every 7 days for transparent dressings.
