During observations on 1/20/2026 and 1/21/2026, Resident 17 was seen eating lunch with a regular spoon from the meal tray. On 1/22/2026, COTA 1 verified that the lunch tray did not have built-up utensils and stated the resident refused them. On 1/23/2026, LVN 2 observed two regular spoons on the tray and stated the resident always had regular spoons and had never been seen using a special spoon, while also verifying the order for built-up utensils had been in place since 12/12/2025. The RD stated the refusal had not been communicated to her, and the DON stated the order was not implemented during meals and that the rehabilitation department should have communicated the refusal and re-evaluated the device's appropriateness. The facility policy stated adaptive devices will be provided for residents who need or request them and assistance will be provided to ensure residents can use and benefit from special eating equipment and utensils.

The facility policy on Adaptive Feeding Equipment stated dietary must be notified of adaptive equipment needs and ensure the equipment is placed on the tray at each meal.
