Summary

The facility failed to ensure that Resident 4’s call light was within reach. Resident 4 was admitted with dementia, muscle weakness, and aphasia, and the MDS indicated severely impaired cognitive skills for daily decision making, partial/moderate assistance with personal hygiene, and dependence for toileting hygiene. The care plan identified the resident as at risk for falls related to confusion, history of falls, incontinence, unaware of safety needs, and poor vision, and also stated to ensure the resident’s call light was within reach and encourage use of it for assistance as needed. The care plan initiated on 5/28/2025 stated that Resident 4 could not verbally communicate, making the call light at the resident’s side very important, and that activities would trigger the call light for medications and pain management. During observation on 1/20/2026, the resident’s call light was seen coiled on the right side of the bed rails and hanging out of reach. During a concurrent observation and interview on 1/22/2026, CNA 1 observed the resident looking for the call light and stated it was coiled at the bed rails, hanging down, and unable to be reached. LVN 1 later stated the call light should be placed within the resident’s reach and was the resident’s way of communicating needs to staff. The facility policy on answering the call light stated that the call light should be accessible to the resident when in bed, from the toilet, from the shower or bathing facility, and from the floor.
