For Resident 64, the admission record showed the resident was admitted on 09/07/2022 with diagnoses including acute respiratory failure, kidney failure, and atrial fibrillation. The H&P dated 1/23/26 indicated the resident had capacity to understand and make decisions, and the MDS dated 12/9/2025 indicated the resident was cognitively intact.
