Pancake Style Call Light Did Not Remain Activated

Summary

The facility failed to ensure that the pancake style call light in one resident’s bathroom and bathing area was functioning properly. Resident 44 was admitted with diagnoses including hemiplegia, hemiparesis following cerebral infarction affecting the right dominant side, and Alzheimer’s Disease. The resident’s MDS indicated severe cognitive impairment and dependence for transfers, bed mobility, dressing, footwear, and personal hygiene. Resident 44’s care plan identified that the resident was rarely or never understood, could not reliably use a standard call light because of cognitive limitations, and had been provided a pancake style call light. The care plan also included an intervention to ensure the pancake call light was clearly visible, functional, and appropriately positioned. A separate care plan entry noted an ADL self-care and mobility deficit related to stroke and indicated that the pancake style call light was to be in place. During observation, the pancake style call light was seen on the resident’s left side and would turn on when pressed, but it turned off when pressure was released. The AD, PR, LVN, DON, MS, and ADON all observed or discussed that the call light required pressure to be held on it to remain on, and they stated it should stay on after being pressed. The DON stated the call light was not functioning properly because it would not stay on when pressure was released, and the facility policy reviewed by the DON and ADON stated that if a call light malfunctions it should be reported promptly and the resident provided with a call bell and close staff monitoring until it is fixed.
