The facility failed to implement, review, and re-evaluate care plan interventions intended to prevent falls for two residents. Resident 20 was admitted with diagnoses including muscle weakness, gastrostomy, and encephalopathy, and the history and physical indicated he did not have the capacity to understand and make decisions. His BIMS score was 9, indicating moderate cognitive impairment. The record showed three falls within 23 days, with change-of-condition notes dated December 28, 2025, January 5, 2026, and January 20, 2026. Resident 86 was admitted with diagnoses including healing left femur fracture, history of falls, cerebral infarction, and dementia. The history and physical indicated intermittent capacity to make decisions, and the BIMS score was 0, indicating the resident was rarely or never understood. The record showed five falls within 44 days, with change-of-condition notes dated December 12, 2025, December 28, 2025, December 29, 2025, December 31, 2025, and January 25, 2026. There was no documented evidence of a care plan update for the falls dated December 12, 2025, and December 31, 2025, and no documented evidence that IDT meetings were conducted for the falls dated December 12, 2025; December 31, 2025; and January 25, 2026. The RN and DON stated that care plan updates and IDT meetings should have occurred for these falls.
