Infection Control Failures During Resident Care and Medication Preparation

Summary

The facility failed to maintain an infection prevention and control program during multiple observed care activities involving residents with enhanced barrier precautions, incontinence care, blood glucose monitoring, and medication preparation. Resident #72 was a male with a feeding tube, bowel and urine incontinence, and a care plan requiring enhanced barrier precautions. During a check and change, CNA F and RA D entered his room without hand hygiene or gowns, performed incontinence care while wearing gloves, and left the room without hand hygiene before going to another resident's room. Resident #73 was a severely cognitively impaired female who was dependent for all ADLs, frequently incontinent of urine, always incontinent of bowel, and received all nutrition through a feeding tube. During observed care, CNA F and RA D used hand sanitizer outside the room and entered without gowns despite enhanced barrier precaution signage. The DON entered, turned off and disconnected the G-tube feeding, then left after hand hygiene. CNA F then provided incontinence care while wearing the same gloves, did not clean the resident's perineal area before turning her, and later reused the same gloves while applying barrier cream. CNA F and RA D again left the room without hand hygiene and proceeded to another resident. Resident #79 was cognitively intact, always incontinent of bowel and bladder, and high risk for pressure ulcers. CNA F and RA D entered for a check and change without hand hygiene, used the same gloves while removing the old brief and placing a new one, and applied barrier cream with the same gloves. For blood glucose monitoring, LVN E and LVN B wiped the glucometers with alcohol prep pads instead of the facility's germicidal or bleach wipes and did not follow the manufacturer's instructions requiring validated disinfectant wipes and proper wet contact time. During medication preparation, MA I used gloved hands to pop tablets from blister packs into her hand before placing them in a medication cup and then used the same gloved hand on the computer mouse and keyboard while documenting medications.
