Infection Prevention and Control Program Deficiencies

Summary

The facility failed to implement effective infection prevention and control practices in multiple areas. During review of the Infection Control Surveillance binder with the Infection Prevention Nurse, the binder for August, September, and October 2025 did not contain an infection trend tracker with changes and resolutions, analysis of identified trends, or educational programs based on prevention, trends, and outcomes. The Infection Prevention Nurse stated the tracking and trending information was not in the surveillance binder, even though the facility policy required ongoing tracking, trending, monitoring, and analysis of infections. The facility also had environmental and laundry-related infection control issues. In the laundry room, three large yellow dirty linen containers had rips on all edges exposing the metal frames, and the Housekeeping Lead stated they needed to be replaced. In the hallway, a housekeeper was observed mopping with plain water and using the same water to rinse the mop for three to four rooms before changing it. The Housekeeping Lead stated management had told staff not to put disinfectant into the mop water and that floors were cleaned with hot water only, while the Administrator stated the water for mopping needed to be mixed with disinfecting liquid. The facility policy required floors to be disinfected with an EPA-registered disinfectant. Resident care practices also showed breaches in infection control. For a resident on contact isolation for ESBL, one CNA removed meal tray items from the room while wearing soiled gloves and handed the tray contents to another CNA in the hallway without changing gloves or placing the items in a plastic bag barrier; the second CNA was not wearing gloves. The Infection Prevention Nurse stated the tray should have been placed inside a plastic bag to prevent cross-contamination and that there was a breach in the infection control process. In addition, two residents were given meal trays without being offered hand hygiene or a towelette beforehand, and the CNA acknowledged that hand hygiene should have been offered. The facility policy stated hand hygiene is the primary means to prevent the spread of infections and that residents should be encouraged to practice hand hygiene.
