Infection Control Lapses With Gloves, Linen Cart Access, and Hand Hygiene

Summary

The facility failed to ensure that CNA 1 wore gloves when handling soiled linen. During an observation in a resident room, CNA 1 was seen handling soiled linen without gloves. In an interview the next day, CNA 1 stated gloves should have been worn when handling soiled linen and explained that wearing gloves helps prevent the spread of infection to other residents. RNS 1 also stated that gloves should be worn while handling soiled linen to prevent the spread of bacteria or cross-contamination, and that not wearing gloves and not performing hand washing after handling soiled linen would create a risk for infection and the spread of bacteria to other residents. The facility policy titled Personal Protective Equipment Gloves stated gloves shall be worn when there is anticipated contact with contaminated items. The facility also failed to ensure clean linen carts were accessed only by facility staff and that staff maintained hand hygiene after touching the trash can lid to open the trash receptacle. During an observation in the hallway near a resident room, a visitor was seen accessing linen from the clean linen cart without staff assistance. CNA 5 stated clean linen carts should only be accessed by facility staff to prevent the spread of bacteria, and RNS 1 stated staff assistance was important to prevent cross-contamination. In the kitchen, staff including the DFS were observed washing their hands and then touching the trash can lid to dispose of trash. The DFS stated staff used a clean paper towel to open the trash can, but acknowledged this practice could lead to cross-contamination and recontamination of hands. The DON stated she was not aware the kitchen trash was not hands-free and that this posed a risk for staff to inadvertently contaminate their hands after washing them.
