Summary

The facility failed to obtain physician orders before initiating low air loss (LAL) mattress therapy for two residents. Resident 2 was admitted and later readmitted with diagnoses including dementia, sepsis, and encephalopathy. The H&P dated 12/1/2025 stated Resident 2 did not have the capacity to make decisions, and the MDS dated 12/23/2025 indicated severe cognitive impairment, dependence for ADLs including oral hygiene, toileting, showering, and positioning, and risk for pressure ulcers/injuries. On 1/21/2026, Resident 2 was observed in bed with a LAL mattress in place and operating. Resident 14 was admitted with diagnoses including malignant neoplasm of the brain, epilepsy, and acute kidney failure. The H&P dated 12/11/2025 stated Resident 14 could make needs known but could not make medical decisions, and the MDS indicated moderate cognitive impairment, dependence for ADLs including toileting, showering, dressing, and positioning, and risk for pressure ulcers/injuries. On 1/21/2026, Resident 14 was observed in bed with a LAL mattress in place and operating. During a 1/22/2026 interview and record review, LVN 3 found no physician orders for a LAL mattress for either resident and stated an order was required before initiating the mattress. The ADON later stated a physician order was required for a low air loss mattress and that placing it without an order was outside the nursing scope of practice. The facility policy stated LAL mattresses shall be used when clinically indicated and ordered by a physician or other licensed prescriber.
