Summary

The facility failed to review, update, and revise the care plans for two residents to address use of a Low Air Loss (LAL) mattress. Resident 2 was admitted and later readmitted to the facility with diagnoses including dementia, sepsis, and encephalopathy. The H&P stated the resident did not have the capacity to make decisions, and the MDS indicated severe cognitive impairment, dependence on staff for ADLs such as oral hygiene, toileting, showering, and positioning, and risk for pressure ulcers/injuries. Resident 14 was admitted with diagnoses including malignant neoplasm of the brain, epilepsy, and acute kidney failure. The H&P stated the resident could make needs known but could not make medical decisions, and the MDS indicated moderate cognitive impairment, dependence on staff for ADLs such as toileting, showering, dressing, and positioning, and risk for pressure ulcers/injuries. During a concurrent interview and record review, the care plans for Resident 2 and Resident 14 were reviewed and the skin breakdown care plan was not revised to include interventions for LAL mattress use. The LVN stated the care plan should have been revised when the LAL was put into use and that care plans serve as a guide for nursing staff to ensure patient-centered care and interventions are not missed. The DON stated a care plan is a blueprint for the resident's care, needs, interventions, and goals, and that if a care plan is not revised, residents may not receive complete and updated care. The facility policy stated care plans are revised as resident conditions change and that the interdisciplinary team must review and update the care plan when there is a significant change, after readmission, and at least quarterly; the LAL mattress policy stated nursing staff and the wound care nurse will incorporate LAL mattress use into the comprehensive care plan.
